Virtual Care

While all Caraday Home Health patients receive in-person visits from clinicians, care can be supplemented with virtual visits and telemedicine technology to offer another level of personalized care. A Caraday team member will facilitate the video meeting with the patient, providing the real-time ability to note changes in condition and offer immediate clinical care.

Social Services

Social services can help improve patient care and reduce the need for medical services by enhancing coordination of care, eliminate duplication, and helping patients and caregivers more effectively manage health conditions. Our medical social workers help patients and their families cope with the emotional and social responses to illness, disease, or condition as well as their physician-prescribed treatment plan.

Caraday Home Health is an integral part of our Caraday Safe Discharge Home program which manages the transition of care from the discharging medical provider during that critical period between discharge and resumption of care under the primary medical provider. Caraday Home Health is a foundational step in a system-wide approach to a long-term and post-acute continuum of care focused on outcomes. Data will be compiled to demonstrate effectiveness in reduced readmissions and lower care costs.

The convenience of home health care is further augmented with options for telemedicine and remote patient monitoring, further extending your clinical team. This continuous care experience provides additional assurance and immediate access using the latest technology for virtual health care.

For care providers such as hospitals, skilled nursing facilities, assisted living communities, Caraday Home Health supports your patients in their road to recovery at home. Outcome-driven, the goal is to provide the services and care your patients need to achieve optimal health.

Caraday Home Health is an integral part of our Caraday Safe Discharge Home program which manages the transition of care from the discharging medical provider during that critical period between discharge and resumption of care under the primary medical provider. While in our care, Caraday Home Health communicates with the referring facility on patient progress as a key partner of your team.

All patients who are referred to Caraday Home Health return to their referring community.

Paying for care

Home health care may be paid for by some insurance providers, as well as by Medicare for those who qualify.  A Caraday team member will help verify insurance and determine eligibility. Patients with Medicare are covered if they meet the following requirements:

  • Must be under the care of a physician and receiving services as part of a care plan created and reviewed by a physician
  • Certified by a physician that they need any of the following services:
    • Intermittent skilled nursing care (other than drawing blood)
    • Physical therapy, speech language pathology, or occupational therapy that are necessary for recovery from an illness or injury or as part of ongoing care
  • Homebound (inability to leave the home without assistance)

Answering your questions

What is home health care?
Home health care is designed to help you recover in the comfort of your home through skilled nursing and physical, occupational, and speech therapy. Your health care provider can order home health if your condition requires you to remain in the home for rehab and recovery from illness, injury, or surgery.

What is the difference between home health care and personal care services?
Home health is skilled clinical care ordered by a health care provider. These services might include things like nursing, physical therapy, and occupational therapy and are designed to help you manage your condition and regain your independence after illness, injury, or surgery.

Personal home care assistance involves non-clinical services, such as help around the house, transportation, bathing, and light meal preparation.

Will Medicare or insurance cover home health care services?
Home health may be covered 100% by Medicare for those who qualify, as well as some forms of insurance. Part of the process includes verification of insurance and determining qualifications for Medicare.  Our team is happy to help answer your financial and insurance questions.

Do you provide home health care services for someone who resides in an assisted living facility?
Yes, we do. We’ll work closely with your physician or other authorized practitioner to develop and strictly follow your plan of care.

Do I need a referral from my primary health care provider to get home health care?
Yes, you should speak with your primary care physician, nurse practitioner or other health care provider. You can also talk to one of our nurses 24/7 at 855-236-7570 or to find out if home health care may be right for you.

If I am receiving home health for my medical condition, can I also get assistance with daily activities such as housekeeping and transportation?
Personal home care is often needed alongside home health services, and we can help coordinate the two levels of care. A limited number of your personal home care hours may be covered by Medicare while you’re receiving clinical in-home care, but it’s generally paid separately or covered by private insurance or other government programs.

What can I expect on the first home health care visit?
Before any treatment begins, a clinical assessment will direct the development of a personalized care plan. As part of the clinical assessment, a clinician will go over your medical history, review prescriptions, and perform a physical exam. Once that’s complete, your personal care plan will be created to best guide you to reaching optimal health.

All visits will be scheduled in advance. During each appointment, which lasts about 45 minutes to an hour, the home health team will work on specific therapies or skills, as well as provide the resources and tools to keep moving forward with your treatment plan.

Can I ask for Caraday Home Health by name?
